Whether you join as a member or come visit by visit, primary care at the practice covers the same ground: preventive visits, chronic condition management, sick visits and follow-up.

Preventive care planning helps you stay ahead of issues. For long-term conditions such as blood pressure and metabolic health, follow-up is steady and structured, with clear communication and continuity between visits.

Choose how you access it: Direct Primary Care Membership for ongoing, relationship-based care, or insurance-based fee-for-service visits. Not for emergencies: call 911 if you are experiencing a medical emergency.
